HPI: Ms. Watkina was diagnosed with Type II diabetes 1 year ago.
PMH: Carpel tunnel surgery (2006), migraines (since 2003).
FH: Father died of complication of diabetes mellitus. Mother alive and well at 64 years of age. Sister with type 2 diabetes at age 35.
SH: Nonsmoker, occasional social drinker (1-2 drinks per night). Married 3 years.
Meds: Metformin 500 mg BID, Maxalt 10 mg PRN, multivitamin QD
ALL: Penicillin - rash, Sulfa - rash
PE:
Gen: WD, LBF in NAD; 5'10", 330 lbs, BP 132 mmHg/82 mmHg, HR 70
HEENT: NC/AT, EOMI, PERRLA, (-) JVD, nasopharynx clear
CV: S1, S2, normal rate and rhythm
Lungs: Clear to auscultation, + bilateral breath sounds, no RRW
Abdomen: + BS, NT
Ext: (-) C/C/E
Neuro: A and O x 3/3, strength 5/5
