The monitoring tools can be used to follow your home self-management of asthma, diabetes, and high blood pressure. This site allows you to track your numbers, relates the numbers to how you are doing and make this information available to your health care providers.
Register (for free) and get started monitoring your home self-care. Have your helath care providers register and you can give them access to your information. You control the system and only those providers that you want to have access to your information will have access.
