The monitoring tools can be used to follow your home self-management of asthma, diabetes, and high blood pressure. This site allows you to track your numbers, relates the numbers to how you are doing and make this information available to your health care providers.

Register (for free) and get started monitoring your home self-care. Have your helath care providers register and you can give them access to your information. You control the system and only those providers that you want to have access to your information will have access.

Imagine that you aren't feeling quite well...you call your doctor and they ask you how you are doing with your blood pressure or peak flow or blood sugar...now, they can look up the information on the site and have a much better understanding of what is going on at home.

Take advantage of this site. It is provided free of charge to help improve communication between patients and their doctors, nurses and pharmacists!
