By using this site, you agree to these conditions and terms. If you do not agree to all of these conditions and terms, do not use this site!

The Ohio Northern University College of Pharmacy will modify, revise, and update these conditions and terms at anytime, as needed.  By using this site (ecivon.onu.edu, ecivon, ecivon.info, ecivon.com, ecivon.net, ecivon.org), you accept these modifications, revisions, and updates.

Health care professionals may direct certain children to use this site as an informational content source.  The informational content entered into the system (personally identifiable information) by any person, with a valid Username and Password, of any age, is available only to that person or to health care professionals designated by that person.

Violation any of any of the Conditions and Terms of Use, automatically and immediately terminates your permission to use this site (Ecivon).

MEMBERSHIP

Certain sections of this site (Ecivon) require a valid User Membership (membership) to access and use, including but not limited to the Monitoring Tools. A valid membership may be obtained by going to the "Register as a New User" page and following the instructions for signing up as a new member.

There is currently no cost for a membership. The cost for registering as a new user is subject to change without notification. Persons who have a valid membership will be notified no less than 30 days ahead of time, by electronic mail (e-mail), if there is to be a change in the cost of their membership. Persons with a valid membership may cancel their membership at any time.

If you wish to cancel your membership, you may do so at any time by sending your cancellation request to the Ohio Northern University College of Pharmacy, via e-mail at d-kisor@onu.edu or regular mail to: David F. Kisor, B.S., Pharm.D., Ohio Northern University College of Pharmacy, 525 South Main Street, Ada, Ohio 45810. Please include your name and your Username, and your request to cancel your membership for this site.

LIABILITY OF THE OHIO NORTHERN UNIVERSITY COLLEGE OF PHARMACY

The use of this site is at your own risk.  When using this site, information content will be transmitted via electronic medium, which is not under the control and governance of the Ohio Northern University College of Pharmacy.  Therefore, appropriately, the Ohio Northern University College of Pharmacy assumes no liability in relation to corruption, failure, delay or interruption of any data or other informational content transmitted in connection with use of this site.

This site and its informational content are provided on an "as is" basis. SUBMISSIONS OF INFORMATIONAL CONTENT TO THIS SITE

You agree that you will not transmit (upload) informational content or any communication of any type that violate any rights of any person or party.  Upon any submission, you automatically express the Ohio Northern University College of Pharmacy an irrevocable, royalty-free, perpetual, global (world-wide) nonexclusive license to use, modify, edit, translate, distribute, perform and display the submission or informational content in any media or medium, or any form now known or developed hereafter, in a confidential manner.  Data submitted to the Monitoring Tools (see item "MONITORING TOOLS (SELF MANAGEMENT)" below) will be held in highest and strictest confidence. The Ohio Northern University College of Pharmacy reserves the right to edit and or remove inappropriate "off topic" postings in the forums.

MONITORING TOOLS (SELF MANAGEMENT)

The monitoring tools provided on this site are a service that allows you to document health information on a limited basis.  These monitoring tools are used on a voluntary basis and are solely to provide informational content to help you better self manage your disease. The informational content (data) you provide using the monitoring tools are stored on servers maintained on behalf of the Ohio Northern University College of Pharmacy. The monitoring tools are structured to allow "writing access" to users with a valid membership, as set forth in the item "MEMBERSHIP" (above).  Submission or editing of your data results in your information being sent over the Internet.  The information you provide is password protected and can only be edited, altered, or appended by you, or by one or more health care providers of your choice.  The Ohio Northern University College of Pharmacy, however, does not warrant or imply or represent that the data you submit or edit using this site is protected against misuse or alteration by a third party.  Nor does the Ohio Northern University College of Pharmacy warrant or imply or represent that the data you submit or edit using this site, will not be lost.  The Ohio Northern University College of Pharmacy has taken steps to prevent the loss of data, however, loss of data is not guaranteed.

You are responsible for ensuring that you protect your password.  You are not to give your Username and Password to an unauthorized individual.  You may provide "reading" access to health care professionals of your choice to enhance communication of informational content.  You may additionally, but are not required to, provide "writing" access to health care professionals of your choice. As set forth in the item "THIS SITE DOES NOT PROVIDE MEDICAL ADVICE" (above), The Ohio Northern University College of Pharmacy and your doctor or health care provider do not monitor the informational content provided by you with the use of this site.  You, however, may make authorized health care providers aware of informational content you provide.  No unauthorized person shall have access to your password or informational content account.  You and only you are responsible for the distribution and use of your password and informational content.  You and only you are authorized to allow "reading" or "writing" access to your informational content.  You and only you can change your password.  You and only you can contact the Ohio Northern University College of Pharmacy to have your password inactivated.

You grant the Ohio Northern University College of Pharmacy and all other persons or entities involved in the operation of this site the right to store, transmit, retrieve, and use your information, in a confidential manner (you will not be identified) in connection with the operation of this site. The Ohio Northern University College of Pharmacy is based in Ada, Ohio, in the United States of America. Access to this site may not be legal by all persons in particular countries.  The Ohio Northern University College of Pharmacy makes no claims that the informational content is appropriate outside of the United States.  Accessing this site from outside the United States is done solely at your own risk.  You are responsible for conformity with the laws of your jurisdiction of your site of access. The following provisions survive the expiration or termination of these Conditions and Terms of Use for any reason whatsoever: Liability, User Submissions, Indemnity, Jurisdiction, and Complete Agreement.

JURISDICTION

You agree that exclusive jurisdiction for any dispute with the Ohio Northern University College of Pharmacy, or in any way relating to your use of this site (Ecivon), resides in the courts of the State of Ohio.  You agree consent expressly to the exercise of personal jurisdiction in the courts of the State of Ohio in connection with any such dispute including any claim involving the Ohio Northern University College of Pharmacy, its officers, directors, or employees.

These Conditions and Terms of Use are governed by the substantive laws of the State of Ohio, without respect to its conflict of principles of laws.  Any provision of these Conditions and Terms of Use found to be invalid by any court having jurisdiction, the invalidity of such provision shall not affect the validity of remaining provisions of these Conditions and Terms of Use. Remaining provisions shall remain in full force and effect. No waiver of any of the Condition and Terms of Use herein shall be deemed a continuing waiver of any other term or condition.

COMPLETE AGREEMENT

Excepting a particular "legal notice" on this site, the Conditions and Terms of Use herein constitute the entire agreement between you and the Ohio Northern University College of Pharmacy
